Fecal microbiota transplantation (FMT) is a therapeutic procedure where fecal matter from a dog with a healthy gut microbiome is given to another dog suffering from a gastrointestinal illness. 

FMT aims to restore the recipient's disrupted natural microbiota, allowing it to stabilize and increase biodiversity. FMT has been a successful treatment in humans for several decades, so it is no surprise that FMT would be attempted in dogs.

The main obstacle to FMT therapy is the "Gross Factor!" Most people are disgusted by the thought of giving their dog poop from another dog! But once the procedure is explained and the benefits are realized, it becomes less intimidating and repulsive.

FMT-like practices were documented in ancient Chinese medicine, where physicians utilized "yellow soup" to treat patients with severe diarrhea. The concept involved transferring fecal material from a healthy individual to someone suffering from gastrointestinal issues. Similar practices involving using fecal material for therapeutic purposes were observed in other cultures, including traditional Ayurvedic medicine in India.

The modern history of FMT began to take shape in the mid-20th century. In the 1940s and 1950s, researchers started exploring using fecal material to restore gut health. Initial studies were conducted to investigate the efficacy of FMT in treating conditions such as pseudomembranous colitis.

Dr. Ben Eiseman, an American surgeon, is credited with performing one of the earliest documented FMT procedures in 1958. He successfully treated four patients with pseudomembranous colitis using fecal enemas. FMT gained attention in the medical literature during the 1970s and 1980s, primarily as a treatment for recurrent Clostridium difficile (C. diff) infections. Researchers observed that introducing healthy fecal material could resolve the imbalance caused by antibiotics and eliminate the pathogenic C. diff bacteria.

FMT experienced a resurgence in interest in the 2010s, especially with the growing awareness of the importance of the gut microbiome in overall health. Researchers explored its potential applications in various gastrointestinal and systemic conditions and 6gained regulatory approvals for treating recurrent C. diff infections in some countries. The recognition of FMT's effectiveness in addressing this specific condition marked a significant milestone.

Ongoing research explores the potential applications of FMT beyond C. diff infections, including its role in inflammatory bowel diseases, irritable bowel syndrome, metabolic disorders, and autoimmune conditions.

Transferring microbial contents from healthy to ill animals has been a long-standing practice with cattle and other ruminants. 

One of the first successful uses of FMT in small animal veterinary medicine was with puppies suffering from parvo, an often deadly viral disease. Treating these puppies with FMT has had similar success rates as with the C. diff. human patients!

However, most veterinarians are not yet comfortable with FMT therapy. Many still feel that it is still too experimental. So, it may be up to you as the pet parent to advocate for this treatment option if you think it will benefit your dog.

The importance of the gut microbiome's role in your dog's overall health is finally being recognized. The gut microbiome affects almost all systems within the body, including the gastrointestinal tract, brain, neurologic system, immune system, and the largest organ, the skin, through the support of many enzymatic and chemical reactions in the body. 

When the gut microbiota becomes imbalanced, many gastrointestinal diseases and disorders can develop, including:

Small intestine bacterial overgrowth (SIBO)

Gastric dilatation and volvulus (GDV or bloat).  

Diarrhea is the most common clinical sign for all of these conditions. Proper digestion and absorption of nutrients are disrupted. Dogs can actually suffer from malnutrition despite being fed daily.

Chronic Diarrhea is the Most Common Reason for FMT

Before recommending FMT for your dog experiencing chronic diarrhea, your veterinarian will attempt to see if the diarrhea is responsive to antibiotics, fiber, and diet. As your dog's advocate, avoid excessive use of antibiotics as it will exacerbate the GI issues. 

If diarrhea continues to be unresponsive, chronic, or recurring, your dog becomes a candidate for fecal transplantation. The gut microbiome of dogs with chronic diarrhea is imbalanced, scientifically known as dysbiosis.

Donor Dogs Need to Meet Certain Requirements

There are no standard guidelines for donor dogs, but veterinarians look for criteria when selecting one. 

Donor dogs must be clear of intestinal pathogens, including:

They must also pass a full wellness exam, including a complete blood count (CBC) and chemistry profile, to rule out any diseases, including cancers, skin disorders, or GI conditions. Donors are not permitted to have taken antibiotics within the past three months or longer. Ideally, the donor would be a dog who has never required systemic antibiotics. 

Dogs born naturally, without the need for a C-section, are also preferred. Exposure to the birth canal microbiome plays a significant role in establishing the initial gut microbiome. Donor dogs should not be fed foods known to cause food allergies or intolerances in the recipient dog for at least 14 days before collecting the sample. 

As with the donor dog, there are no set requirements for the recipient dog. However, the following must be avoided for at least ten days before treatment:

Ideally, the patient's gut microbiome should be tested for an established baseline. Then, it can be tested and compared following the completion of the procedure. 

Samples from the donor dog are collected immediately upon a natural elimination of the feces. The sample is combined with saline, one part saline to four parts feces, and blended. The sample should be transplanted within six hours of collection. 

Because the dog's stomach is highly acidic, which destroys bacteria, it is difficult to dose the fecal material by mouth. However, Animalbiome (animalbiome.com) has developed a "poop pill" that is being used successfully for FMT.  

A tube is passed from the nose or mouth to deliver the prepared fecal suspension to the small intestine directly. Placement of the stomach tube requires sedation and usually involves the use of endoscopy. 

An alternative and more commonly used method is to perform the procedure rectally. Before a rectal transplant, an enema is given to the patient to clear the lower intestines.

The fecal suspension enema is then deposited into the rectum and retained in the colon as long as possible. Forty-five minutes is ideal but challenging. The patient must be manually restrained to prevent them from eliminating the transplanted material. 

You should notice your dog's clinical signs showing improvement within 24 hours if the treatment is successful. Within 48 hours, the composition of your dog's microbiome in their stool should resemble that of the donor dog's.

If you plan to do a follow-up gut microbiome test,  you must wait for at least 48 hours following the procedure.

It is important to note that while FMT may sound like a miracle cure-all, it does not work on every patient. The existing conditions may be too overwhelming for the transplanted bacteria, requiring multiple treatments.

You must help your dog maintain the restored gut microbiome for the transplant to succeed.  

Feeding a healthy, species-appropriate diet with whole, raw,  human-grade food is the best way to maintain the newly restored gut microbiota. Prebiotics and probiotics will help keep the proper balance of microbes in the gut.
